Simpley follow these direction.

Fill out the ORDER FORM providing size, color and quantity
of each item desired. Please remember to include
your shipping information with a phone number.
Shipping cost is $5.95 per order. If you live in the
Milwaukee area, please call and we can arrange a pickup
instead.
Total your order, including shipping. Please make all
